A Forsyth County man has been indicted after authorities say he was drunk and driving with a revoked license when he crossed the center line and crashed head-on into another vehicle, seriously injuring a 74-year-old woman.
According to a Forsyth County Sheriff’s Office report, the crash happened on Nov. 28, 2025, on Majors Road near Big Creek Bridge in Cumming.
Investigators identified the driver as Matthew Thomas Walters, 28. He faces charges including DUI less safe, serious injury by vehicle, habitual violator, failure to maintain lane and possession of an open container of alcohol.
Deputies said they responded to a serious-injury crash and found Walters inside an ambulance receiving treatment for a head injury. According to the report, deputies immediately noticed a strong odor of alcohol and observed signs of impairment, including bloodshot eyes, slurred speech and watery eyes. Walters reportedly admitted he had consumed “a couple of drinks” before the crash. He refused both a portable breath test and a requested blood test, according to investigators.

A witness told deputies he was traveling behind a Honda Accord driven by Pamela Elizabeth Custer, 74, when he saw Walters’ vehicle cross into the southbound lane and strike Custer’s car head-on.
Investigators said evidence at the scene showed Walters was negotiating a curve when he crossed the center line. The report states deputies found no indication he attempted to brake before the collision.
Deputies also reported finding a partially full Fireball bottle in Walters’ vehicle. A records check revealed his driver’s license had been revoked and that he had previously been declared a habitual violator. Investigators also noted prior DUI convictions.
The crash left Custer with serious injuries. According to the report, she suffered fractures and was rushed into surgery because doctors feared internal bleeding. Custer told investigators she did not remember the crash and only recalled waking up in the hospital. Her husband told deputies he was on the phone with her just before the collision and heard her cry out before the line disconnected.
Authorities later secured warrants charging Walters with DUI less safe, possession of an open alcohol container, habitual violator, failure to maintain lane and serious injury by vehicle.
